Disclaimer-

In consideration of being allowed to participate in any way in the Unparalleled Lacrosse Programs, related events, activities, clinics, lessons, or camps, the undersigned acknowledges, appreciates, and agrees that:

1.         The risk of injury from activities involved in this sport are significant, including the potential for permanent paralysis and or death, and while particular rules, equipment, and personal discipline may reduce the risk of injury, the risk of injury does exist. All boys are required to use NOCSAE certified equipment, including helmet, arm pads, gloves, and shoulder pads. Girls are required to wear NOCSAE certified eye protection.

2.         I knowingly and freely assume all such risks, both known and unknown, even if arising from negligence of releases or others, and, assume full responsibility for my or my child’s participation.

3.         I willingly agree to comply with the stated terms and conditions for participation and agree to abide by the equipment regulations likewise stated. If I observe any hazard during my presence at an Unparalleled Lacrosse Event or Program, I will remove myself from participation and bring such hazards to the attention of the Event Director.

4.         I, for myself, and on behalf of my heirs, assigns, personal representatives, and next of kin, herby release and hold harmless Unparalleled Lacrosse. LLC, their officers, employees, members, directors, coaches, subcontracted employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“releases”) with respect to, any and all injuries, disability, death, or loss or damage to person or property, whether arising from the negligence of the releases or otherwise.
